February 7, 2014 (Lamar, CO) – Rehabilitative care is often an essential component of the medical treatment plan to prepare an individual for a return to their home environment. Juniper Village at Lamar knows how important privacy can be to a patient who needs to enter into a nursing home for this therapy and created a special program and room called Bridge to Home to accommodate their wellbeing. The program provides a living environment with resources and therapy to facilitate the patient’s independence and provide a path home after hospitalization.

Recently, Ken “John” Gause entered Juniper Village at Lamar for a short-term stay in the program. When it was time for him to move out, he was so comfortable in his new space that he asked to remain there. In speaking with Juniper, his family decided to generously donate the funds to create a new Bridge to Home room so that their father could remain living at Juniper Village.

“We were delighted that Mr. Gause wanted to stay with us and that his family provided the solution. Our entire philosophy is based on fostering healthy independent lives for our residents and promoting a sound mind, body and spirit,” expressed Sharon Fulton, Executive Director. “We welcome him as a permanent member of our family. I don’t think there could be a better testimonial than this.”

The private rooms provide a homelike environment. They are located in a quiet area away from all the hustle and bustle of the nursing home and equipped with a large screen TV, recliner and private bathroom. Housed in Juniper’s therapy center, patients have easy access to physical therapy, occupational therapy, and speech therapy. Again, typically the rooms are designated for short term Medicare stays for individuals who will return to the community after their therapy is complete.

The goal of the Bridge to Home rehabilitative care program is to treat and educate the patients regarding functional performance, exercise programs, adaptive equipment, pain management, home safety and caregiver training. Juniper welcomes visitors to take a tour of the building and see the fabulous room.

Juniper Village at Lamar is located at 205 S. 10th Street, telephone (719) 336-3434.
