Patient Centered Medical Home Upgraded

High Plains Community Health Center has been notified of its Recognition by the National Committee for Quality Assurance (NCQA) as a Level III Patient Centered Medical Home based on 2011 Standards. High Plains was Recognized under 2009 standards in February of 2011. Since that time NCQA issued more stringent upgraded standards that High Plains has also met and exceeded.
