Taking Charge of Your Health Through Self-Management of Chronic Disease Workshop

Southeast Health Group LogoThe Health Navigators at Southeast Health Group are here to assist community members of Southeast Colorado “take charge of your health”.  The Health Navigators have been trained through the Consortium for Older Adult Wellness in Lakewood, Colorado to provide self-management of chronic disease workshops developed by the Stanford University School of Medicine to community members who want to learn how to become better managers of their health care.  The workshops are free of charge and care givers are welcome to attend as well.

People who have completed the workshop have demonstrated significant improvements in:

  • general health;
  • increased exercise;
  • feeling more energetic;
  • a better understanding of their health condition(s);
  • improved communication with health care providers.

With those improvements, participants have also spent fewer days in the hospital and required less outpatient visits as well!

Workshops are currently scheduled to take place this spring and summer in Holly, Walsh, and Eads.  They run for about two and half hours, once a week, for six weeks.  Workshops cover the topics of self-management of chronic disease or self-management of diabetes.

If you, or someone you care for, have a chronic condition such as Diabetes, Depression, Arthritis, Heart Disease, Asthma, Bronchitis or Emphysema, give a Health Navigator a call to reserve a seat at one of the up-coming workshops.  A copy of the companion book, “Living a Healthy Life with Chronic Conditions” will be provided to each participant.
