Juniper Village at Lamar is a skilled nursing facility and home to 50 residents on average. Juniper Village at Lamar delivers a wide range of care and services, from respite care to short-term rehabilitation to skilled care and long-term residency. Short-Term Rehabilitative Care: The Bridge to Home program is a joint effort between Juniper Village at Lamar and Genesis Rehab Services.  The program provides individuals with clinical expertise in an environment that ensures a smooth transition from acute hospital stay to rehabilitation and finally to return to home and community.

The Bridge to Home program is a specially designated area at Juniper Village that includes a newly renovated therapy center with state of the art equipment. Also newly renovated for Bridge to Home clients, Juniper Village offers a group of separate, handicap accessible private and semi-private accommodations with its own dining and living areas for program participants. This program offers:

• Physical therapy to restore normal movement and function, improve strength and balance, and regain mobility and coordination.

• Occupational therapy to increase independence in all activities of daily life, improve perception, reduce swelling and pain, and control spasticity.

• Speech therapy to improve communication skills, treat language impairments and cognitive disorders, evaluate and treat hearing loss, and assess and treat swallowing difficulties.

Long Term Care: Our focus for our long-term residents is on extending quality of life and providing care and service tailor made to the individual. Of course Long-term residency offers also access on an as needed basis to the above services. This includes:

• Development of an individualized service plan based on physician recommendations, any need for assistance with activities of daily life, our application for residency, and My Life Story (Juniper’s proprietary profile)

• A warm, comfortable environment which provides safety and security and a sense of home; our community was recently renovated with a country kitchen, sun room, lovely dining room, shady outdoor veranda.

• Certified Nursing Assistants – dedicated, highly trained staff provide quality care and service.

• Home-style meals, with the ability to accommodate special diets.

• Transportation throughout the community and to physician appointments.

• Excellent housekeeping, laundry and maintenance services

Connections: Juniper’s social activities program is designed to keep residents connected, physically active, engaged and involved with others, and connected to the community.
